DPC participation as a Liberty HealthShare member

Liberty HealthShare encourages our members to explore Direct Primary Care (DPC) practices to receive the best healthcare service at the best price. A DPC membership can be a great way to manage your healthcare needs, possibly giving you access to same-day appointments, direct communication with your provider, telemedicine visits, and more!

What makes a Direct Primary Care Practice?

There is no single, legal definition of what makes a practice a Direct Primary Care practice, but there are certain characteristics which Liberty HealthShare uses to determine if a practice is a DPC practice in accordance with our Sharing Guidelines. The below facets and features are what distinguish a DPC provider from something like an Annual Retainer or Concierge Practice.

A DPC operates on membership-based payment through monthly, quarterly, or annual membership fees. Most or all primary care services are included and there is no copay for covered services.
A DPC practice DOES NOT bill commercial insurance, Medicare, or Medicaid for primary care services included in membership.
A DPC membership includes, but is not limited to, office visits, annual wellness exams, chronic disease management, acute illness visits, preventative care, and basic office procedures.
A DPC practice offers most, not all, of these features: same-day or next-day appointments, longer appointment times, direct communication via phone or text, telemedicine visits, and/or minimal waiting times.
A DPC physician cares for a smaller group of patients, allowing for more personalized care.
Services not included in your DPC membership are offered at disclosed, negotiated rates.

Members of Liberty Unite, Liberty Connect, and Liberty Essential have the opportunity to share up to 75% of their Direct Primary Care membership fees with the Liberty HealthShare community, up to the maximum monthly amounts of $50 for a single, $75 for a couple, and $100 for a family, based on their program enrollment. Any applicable sharing program Co-Share must be paid by the member and is not eligible for sharing.

For example, a couple enrolled in Liberty Connect, which has a 10% Co-Share, are enrolled in a DPC program with a monthly fee of $120.

Up to 75% of the DPC membership fees are eligible for sharing, and 75% of $120 is $90. However, the maximum monthly amount eligible for sharing for a couple DPC membership is $75, so that maximum sharing limit is applied. Finally, the Liberty HealthShare sharing program Co-Share must be applied if applicable. In this example, the 10% Co-Share of Liberty Connect would be applied ($75 * 10% = $7.50), meaning the final eligible sharing amount for this request is $67.50 ($75 - $7.50).

Liberty HealthShare’s New DPC Incentive for Providers

As another way we can offer our members freedom of choice and the power to make informed healthcare decisions, Liberty HealthShare has created a new incentive program for DPC providers to help their members enroll as Liberty HealthShare members.

You assist a patient in becoming a member of one of three Liberty HealthShare sharing programs designed to meet a variety of needs and budgets.
Your patient, if accepted as a Liberty HealthShare member, completes the DPC Reimbursement Request Form.
Members may receive sharing from the Liberty HealthShare community up to 75% of DPC Fees, up to a maximum amount determined by their sharing program, and subject to the member's Co-Share, if applicable.
After they have been a member in good standing for at least 90 days, you are rewarded with a $500, one-time incentive for each Liberty HealthShare membership you help enroll.
