Liberty HealthShare’s New DPC Incentive

As another way we can offer our members freedom of choice and the power to make informed healthcare decisions, Liberty HealthShare has created a new incentive program for DPC providers to help their members enroll as Liberty HealthShare members. 

Here's how it works:
  • You assist a patient in becoming a member of one of three Liberty HealthShare sharing programs designed to meet a variety of needs and budgets.
  • Your patient, when accepted as a Liberty HealthShare member, completes the DPC Reimbursement Request Form.
  • Members can receive sharing from the Liberty HealthShare community up to 75% of DPC Fees, up to a maximum amount determined by their sharing program.
  • After they have been a member in good standing for at least 3 months (90 days), you are rewarded with a $500, one-time incentive for each Liberty HealthShare membership you help enroll.
